Lung or heart-lung transplantation is the treatment of choice for several end-stage parenchymal and vascular pulmonary and cardiopulmonary diseases that are refractory to conventional medical and surgical therapy. This edition of "Recent Insights and New Perspectives in Lung Transplantation" will benefit from the contribution of speakers representing several of the most qualified national and international centers in thoracic transplantation. The first two sessions will focus on transplant immunology: chronic rejection will be covered, as well as the most recent clinical trials of new immunosuppressive strategies. Considering the large disproportion between the high demand and low availability of suitable lungs, the third session will look at the evaluation criteria of marginal donors and the potential risks associated with their use. The fourth session will highlight the Italian experience. After a general overview of the national program, the session will focus on lung transplantation in cystic fibrosis patients, where the pulmonary involvement is a major limiting factor in survival and quality of life. The fifth session will be entirely dedicated to pulmonary hypertension, with particular attention to Eisenmenger's syndrome, idiopathic pulmonary hypertension, and chronic thromboembolic pulmonary hypertension. Current surgical interventions will be addressed. Intra- and postoperative management of the surgical patient with pulmonary vascular disease will be covered, highlighting distinct features associated with each procedure.
